As part of our services, Anam Cara House Colac provides a palliative in-home respite service and community liaison nurse. Our nurses and patient care attendants offer support to people who have a chronic life-limiting illness and are in the last 3 months of life.
The aim of the respite service is to:
- Support people to have a choice in how and where they die
- Offer reassurance for carers
- Be available to pre-assessed, eligible individuals and their families for daytime and overnight support
- Assist individuals and families if they have an overnight emergency
- Help individuals and their families to achieve a comfortable death at home if this is their wish
How to access our services
People can be assessed for the service by:
- Self-referral
- Health provider
- Referral by next of kin or family member
